Specifically, I am requesting access to: 1. Copies of all inspection reports carried out at Sean Ross Abbey by or on behalf of the Department of Health. 2. Details of who carried out those inspections (for example, departmental inspectors or medical officers). 3. Records showing who those reports were sent to within the Department or elsewhere in government. 4. Any replies, follow up actions, or recommendations arising from those inspections. 5. All records, reports, or correspondence relating to Dr. Margaret Dunleavy, who worked for the Department of Health and was involved in vaccine trials and public health work, insofar as it relates to Sean Ross Abbey. 6. All autopsy records, medical certifications, and related public health files concerning deaths of infants or children at Sean Ross Abbey.
