NEW WORDING: All correspondence (email/letter) between the Chief Nursing Officer and the HSE Chief Clinical Officer in regard to mitigating the risk of adverse outcomes in maternity services in Ireland. The specific adverse outcomes would be maternal death, intrapartum foetal death, early neonatal death, and babies that require therapeutic hypothermia. The correspondence would be from 1 January 2022 to 16 March 2023.
