In consequence I would like, under the Freedom of Information Act 2014, to request (for each of the cases you have in your data base, covering the 2011- 2015 period) the following data: 1. nature of the injury - ex. airway complication, dental trauma, nerve damage, consent issues, psychological trauma, awareness etc. 2. severity of the injury - death, loss of job for the patient, loss of quality of life, permanent nerve damage etc 3. previous claims in the name of same doctor 4. seniority of the doctor involved - NCHD vs Consultant 5. demographics about the doctors (age, sex) 6. outcome: settled in/outside court 7. compensations paid – if it is not possible for each case, a yearly total with a specified minimum and maximum value will suffice
