1) I request access to a copy of the declaration setting out all of the above in relation to the ISPCA: - (see 1 above) a copy of the will and its provisions of Theresa Frances Mcconnell - (see 2 above) details/ instructions given to the solicitor Martina Sheridan who is listed as a witness to the original will, and was also the Administratix - (see 3 above) a copy of the statement provided that the charity named in the will never existed, or details of when it ceased to exist and any evidence provided by the Administratrix, the ISPCA or the CRA to support this statement - (see 4 above) details of all funds available - (see 5 above) all details pertaining to evidence of any special relationship which the deceased had with the ISPCA - (see 6 above) a copy of the form of undertaking completed by the trustees of the charity (ie. the ISPCA trustees) who are to carry out the trusts under the new cy-pres scheme 2) I request access to all information regarding this confirmation and all evidence the CRA has to prove the "significant presence" of the ISPCA in Meath 3) I request access to all information with regards steps taken by the CRA to ascertain the existence of a "Meath branch" of the ISPCA and any evidence found to support its non-existence. 4) I also request access to all correspondence, emails, email attachments, notes of meetings/ phone calls, court documents etc, pertaining to the estate of Theresa Frances McConnell: • to and from ISPCA staff or trustees • to and from Meath SPCA, Registered Charity Number (RCN): 20200862 • to and from • to and from any member of the press or public, or a solicitor representing same
