All documents to include any submissions or objections made, relating to the planning and construction, of the cycle lanes running parallel to Chesterfield Avenue, Phoenix Park, Dublin 2.All documents to include any submissions or objections made relating to the planning and construction of the pedestrian walkways running parallel to Chesterfield Avenue, Phoenix Park, Dublin 3.All submissions, by any group, association, person or any other body or entity made to the Office of Public Works concerning the aforementioned cycle lanes and pedestrian walkways for a period of 01/01/2014 to date 4.Any risk assessments carried out concerning the aforementioned cycle lanes and pedestrian walkways. 5.Records of accidents on the aforementioned cycle lanes for the period 01/01/2014 to date 6.All documentation concerning an accident which occurred on the 2nd May 2016, which occurred on the aforementioned cycle lane, involving a cyclist and a pedestrian. 7.All documentation concerning an accident which occurred on the 1st August 2017 on the roadway of Chesterfield Avenue, between Castleknock Gate and North Road involving a cyclist and a motor vehicle and at which the servants or agents of the OPW were in attendance. Such documentation to include, but not limited to, incident/accident report forms, statements and correspondence. Names of the parties involved may be redacted if necessary.
