Details of how many near-misses or ‘no-harm’ incidents have occurred in maternity wards since 2014. I am seeking these records broken down by year and by hospital and categorised by the nature of the incident ie. in relation to clinical procedures, in relation to blood products etc. If the records are contained in a report of any kind – I would like to receive a copy of this report.
