1 All records in relation to the maintenance works carried out to the damaged sluice outfall / embankment structure on the River Shannon Flood Defence Embankment System in or about August/ September 2019 to include all records relating to a prior risk management assessment, health and safety statement, method statement, scope of works, designers’ risk assessment. 2 All records in relation to actions taken by the OPW its servants or agents to mitigate the risk of flooding on foot of the above maintenance works being undertaken in or about August/ September 2019 including emergency repair works and installation of temporary defences undertaken prior to the breach of the embankment causing flooding to areas of Coonagh and surrounding areas on the weekend 30th August - 1st September 2019 3 All records in relation to remedial actions taken by the OPW its servants or agents to repair the breach of embankment following the flooding to areas of Coonagh and surrounding areas on the weekend 30th August - 1st September 2019.
