Copies (in hard and soft copy format) of all records held created after 4th May 2016 held and/or under the control of the Health Information and Quality Authority in which the National Maternity hospital is mentioned including but not limited to all: 1. Letters; 2. Emails; 3. Memorandum 4. Notes of meetings/telephones conversations; 5. Minutes of meeting/teleconferences; 6. Texts 7. Reports 8. Documents; 9. Correspondence; And any other records exchanged between, held by or under the control of the Health Information and Quality Authority, its officials or agents and any other relevant parties in relation to the National Maternity Hospital.
