I am seeking a copy of the form used by employees to disclose a disability for the purposes of recording disability numbers under Part 5 of the Disability Act 2005. This request is made under the Freedom of Information Act 2014, in particular Section 12, which provides that every person has the right to access records held by a public body. However, if it is possible to provide the form directly without the need to process this as a formal FOI request, I would be grateful if you could do so.
