Copies of relevant information that may have been gathered by the Health Information and Quality Authority when compiling its reports on the conditions in the care centres run by the XXXXXXXXXXX. In order to facilitate the request we shall limit our request to the documentation being sought to that which pertains to Villa Aisling, unless the documentation relates to the practices of the XXXXXXXXXXX care units XXXX in a general fashion. Specifically we require: Complaints in relation to workloads, working conditions, size of the rooms, manual handling of service users, systems of work, weight of the hoists etc. We would specifically request XXXXXXXXXXX. We understand that these XXXXXXX would have been logged in a "House Diary" and would require copies of these diaries. Working policies Details of training provided especially in relation to safe lifting procedures and hoist trainings. Any risk assessment forms, particularly those that pertain to the period prior to May 2011 or any documentation evidencing a decision made to alter practices that would have been in place in or prior to 2011 Details of the operational history and maintenance records of the hoists. Safety Statements
