The decision to conduct a review of Investigations Division (eg: who made the decision and what reasons lay behind it) 2. The selection of personnel to conduct the review , what criteria were applied in this selection and who made the selection? 3. The terms of reference/instructions issued to the reviewers including the dates of all such terms of reference/instructions and the identities of the persons issuing these.
