Please supply a list (anonymised) of all known clinical incidents related to urethral catheter injury to the urethra (male, female, paediatric) from January 2008 until January 2018. A legible, anonymised copy of the original report is adequate. How many of this number were referred to the SCA for the purposes of defending a medicolegal action? How many claims were successfully defended? What was the total cost of managing all claims (successfully defended or not) over the same period.
