Details of the number of patients that had the wrong body part/site/side operated on or treated between 1 January 2012 and 31 December 2016, including the hospital where the surgery took place and the year in which it occurred. 3. Details of the number of patients that underwent the wrong process/treatment/procedure between 1 January 2012 and 31 December 2016, including the hospital where it took place and the year in which it occurred. 4. Details of payments made (compensation/settled claims etc) made to patients in the cases outlined above, including the amount awarded in each case. 5. Details of the Clinical Indemnity Scheme and the overall amount paid out to patients as part of the scheme between 1 January 2012 and 31 December 2016.
