I am interested only in those cases where a claim was paid or investigated based on an incident cased by the anesthetic team. 1. year of incident 2. year of claim 3. nature of the injury/brief description of the case - this is the most important information we are interested in; ex. airway complication, dental trauma, nerve damage, consent issues, psychological trauma, awareness etc. a brief description of the case would help us to identify the nature of the injury 4. severity of the injury 5. outcome: case closed or still open 6. amount paid - a total for all the cases mentioned will suffice. 7. demographics of the patient: age, gender 8. Hospital Trust - where the incident occurred
