Copies of all Department of Health minutes from all structured monthly meetings held between members of the National Women and Infant Health Programme and the Department of Health, on matters pertaining to maternity and gynaecology services, between September 01, 2023 and July 31, 2024 where there is any reference to the establishment of the Confidential Inquiry into Perinatal Deaths that the HSE announced in July of this year. Copies of all quarterly updates between September 01, 2023 and July 31, 2024 that the National Women and Infant Health Programme sent to the Department of Health in order to keep the Minister of Health, and the relevant unit within the Department, up-to-date on service developments and initiatives, which make any reference to the establishment of the Confidential Inquiry into Perinatal Deaths that the HSE announced to the Irish Examiner in July of this year.
