I wish to request the following records under the Freedom of Information Act 2014, relating to the maternity department at Galway University Hospital for the period 1st of January 2023 to 31st of December 2023 (inclusive). 1. All correspondence (letters, briefing notes, reports, emails) between the Department of Health and: •Galway University Hospital, or •The Saolta Hospital Group, Concerning the provision, performance, or governance of maternity services, including but not limited to issues of safety, risk management, or quality of care. This request is intended to capture records relating to safety concerns, but I also wish to include records addressing wider governance and performance issues where relevant. 2.Copies of any reports, audits, reviews, or assessments commissioned or received by the Department concerning maternity services at Galway University Hospital. 3. Minutes and agendas of any Department of Health meetings where maternity services at Galway University Hospital were discussed. 4. Letters or emails to the Minister of Health from service users, relatives, staff, professional bodies, unions or members of the public with concerns regarding the maternity unit in Galway University Hospital. Personal identifiable information is to be redacted from such correspondence.
