I am requesting under the Freedom of Information Act a copy of a report, which I believe to be held by your department, titled something like "Investigation report: the unauthorised accessing of the APS Database". I think the report is dated 30 November 2022, or about that date. Please also furnish me with any briefing notes associated with this report and any referral made to the Data Protection Commission on foot of its findings (or the incident to which it pertains).
