I wish to make a request under the Freedom of Information Act for information held by the State Claims Agency on the National Incident Management System. More specifically I’d like to request information in relation to clinical incidents and serious reportable events within the Health Service Executive (HSE) as follows: · A breakdown of all incidents within the HSE, by incident type, and by facility, recorded on the National Incident Management System between January and June 2016 · A breakdown of all serious reportable events notified by the HSE and recorded on the National Incident Management System, by category (Surgical, Product/Device, Patient Protection, Care Management, Environmental, Criminal), by incident type, and by facility, between January and June 2016 · Data and analysis reports, carried out by the State Claims Agency in 2015, on clinical closed claims in the following areas: Ø Retained foreign bodies in maternity services Ø Retained foreign bodies in gynaecology services Ø Mental health Ø Paediatrics Ø Emergency medicine
